What is EFAST and What’s Changing?

Eko has upgraded the AI algorithm powering your heart sound analysis to EFAST (Eko Fast Algorithm for Structural Murmurs). This next-generation, FDA-cleared technology represents a significant upgrade in both clarity and clinical utility for eligible users.

From "All Murmurs" to "Structural Murmurs" The most important change is how the algorithm identifies murmurs. The legacy Eko+ algorithm flagged any murmur, including "innocent" or "flow" murmurs, which are innocuous sounds caused by blood flow through structurally normal hearts. EFAST is more targeted—it specifically flags structural (pathologic) murmurs, which are more likely to indicate an underlying heart condition. The result is that EFAST flags clinically meaningful murmurs while reducing unnecessary follow-up for innocent murmurs.

Enhanced Rhythm Detection EFAST achieves 95% sensitivity and 94% specificity for AFib detection. Additionally, we have updated our algorithm outputs to provide clearer rhythm classification and reduce unclassified ECG results.

FAQs

What are the new terms for heart rate results?
If an ECG is available, EFAST now uses more descriptive phrasing for heart rate. If the rhythm is normal sinus rhythm, instead of just seeing "Tachycardia" or "Bradycardia," you will see "Sinus tachycardia" or "Sinus bradycardia".
What is an "Unspecified rhythm"?
This term replaces "Unclassified ECG". It simply means the algorithm determined that the rhythm is neither normal sinus rhythm or AFib — not necessarily that something is wrong. If you see this, you can visually inspect the ECG waveforms in the app to determine whether the ECG is clinically important (e.g., heart block) or not (e.g., isolated PVCs).
Why did my result change from "No murmur" to "No structural murmur"?
We updated this label to be more descriptive. "No Structural Murmur" tells you specifically that no murmur associated with a structural heart abnormality was detected. The meaning is the same, just more precise.
Who is eligible for these EFAST updates?
EFAST is automatically available to users in the United States who have an active Eko+ membership, use a Littmann CORE, CORE attachment, or CORE 500 device, and have the Eko App updated to version 5.43.0 or higher.
Why doesn't my DUO device show these new results?
EFAST is currently compatible only with Littmann CORE, CORE attachment, and CORE 500 series devices. DUO devices will continue to receive high-quality analysis through our existing EAS algorithm.
Is EFAST available outside the U.S.?
EFAST is currently FDA-cleared for the U.S. market only. We cannot share a timeline for international availability at this time.
